Pre-Participation Sports Packet

Dear parents, the following forms need to be filled out every year before your child can participate in interscholastic or intramural sports, including track and cheerleading. If you have multiple children, please complete one for each child. If you do not have access to a printer, you can stop by the office to get your packet.

1. Please, print out the sports packet, read, sign, and return to the school nurse.

2. Please, print out the consent for the concussion baseline test and return it filled out to the school nurse.

3. Please, print out the physical packet, fill out the History Form, have your pediatrician fill out the rest, and return to the school nurse.
